The underwear tightens enough and keeps the hernia in place. The pad is purchased separately. The underwear works without the pad for me as it keeps the hernia inside, but I will use the pad when the underwear loosens a bit. I got it in a boxer style that tightens and bothers the thighs a bit, but it has more fabric to keep the pad in place. I didn't get it for after the surgery, but to avoid the pain of the hernia when I walk, while doing natural therapy with patches made from castor oil, and to keep the hernia in place. The pad has a design that doesn't bother during normal walking and doesn't protrude too much. The fabric composition is 87% polyester and 13% elastane. It is breathable for the skin. The underwear has a front opening for easy urination without taking it off. At night, when the hernia is already in place, I wear regular boxers for comfort. Unfortunately, due to pressure and friction on the groin after six days of use, a skin wound was created, so I remove two stars and might stop using it. On the contrary, the special zones do not put pressure on the groin. So, until I receive the hernia belt, I will have to continue with caution
