The name of the Virgin Mary comes from the hymn of the Compline "Spotless, Immaculate, Undefiled..." composed by the monk Paul of the Evergetis Monastery.

The Immaculate One in Orthodox iconography is also called the Virgin of the Passion, because Christ is gazing at the two Angels depicted on either side of the Virgin, holding in their hands the symbols of the Passion, namely the Cross, the Spear, and the Sponge.
